Pour initier votre demande, nous vous invitons à remplir le formulaire ci-dessous qui initiera votre dossier. Nous vous demandons aussi de télécharger le document PDF et dessous et à nous le renvoyer signé. En effet, les signatures des deux parents est nécessaire pour valider la demande de baptême.
